Error 114 - Index does not match the table. Delete the index file and re-create the index.

Take a backup
Open the table in error files
Needs to be exclusive and ctrl y to add a new record,
then mark it for deletion delete it
close out of the screen
Pack the table
Run UDS with re-index
IF unable to open the table, you will need to ASCERTAIN which tables have a corrupt index table 
Take a backup of the company data and manual backup of the SYSTEM folder
Use dataviewer/tools and run the REPAIR TOOL, selecting the COMPANY DATA folder  - ensure to run "CHECK FOLDER" only
This should then give you the tables which have corrupt INDEX (so for example E_WCLNDR)
Then from an OLDOP?? folder or if you have to use Z_???????.cdx replace the corrupted  "CDX" ONLY files (so if you use the Z_WCLNDR.CDX ensure to amend the company letter from Z back to E)
Then log into Opera3 and RUN UDS with reindex
Hopefully no further issues and you can now access the form which was giving the error
